Please ensure that you fill out all the sections labeled as “Required”. The remaining sections are optional, and your coverage cannot be denied if you choose not to complete them.

Finding the Right Plan for You
Healthy Arkansas’ plan serves to bridge the disparities and address expenses not included in your Original Medicare Part A & Part B programs. You qualify for our plan if:
- You live in our Geographic Service Area
- You are a United States citizen (or lawfully present in the US)
- You have both Medicare Part A & Part B
Part A of Original Medicare covers in-hospital stays and services, skilled nursing care, hospice care and certain types of home healthcare.
Part B of Original Medicare covers doctor visits, outpatient care, other types of home healthcare, medical equipment and many preventive health services.
